Please download the new visa application form HERE. The Visa Office of Chinese Embassy notified that starting on September 16, 2011, all applicants are required to use 2011 edition of the "Visa Application Form of the People's Republic of China" and, if necessary, the "Supplementary Visa Application For of the People's Republic of China". The forms shall be fully completed on a computer and clearly printed out. If some items in the form are not applicable, please mark "N/A".
Non-US citizen applicants are required to fill out the V2011B form. Please download the form HERE
